Please read the following waiver and release carefully. By typing your name in the designated fields, you are electronically signing this document and agreeing to the terms herein. I, Parent/Legal Guardian, hereby certify that I am the parent or legal guardian of the Participant, and I am fully authorized to execute this Registration and Waiver on behalf of the Participant. I affirm that the Participant is in normal health and possesses the physical ability to participate safely in the Community Lifestyle Programs/Events. Emergency Medical Authorization: In the event of an emergency, I hereby authorize the directors, employees, volunteers, or agents of Community Lifestyle to act on my behalf and in the best interest of the Participant to secure any necessary emergency medical treatment. I understand that efforts will be made to contact me as soon as possible if such a situation arises. Release and Waiver of Liability: I, for myself and on behalf of the Participant and our respective heirs, executors, administrators, successors, and assigns, hereby release, waive, discharge, and covenant not to sue Community Lifestyle, its subsidiaries, affiliates, parent companies, partners, predecessors, assigns, officers, directors, employees, agents, and volunteers from any and all liabilities, claims, actions, damages, costs, or expenses of any nature whatsoever arising out of or in any way connected with the Participant's involvement in Community Lifestyle Programs/Events, whether arising from the negligence of the Released Parties or otherwise, to the fullest extent permitted by law. Consent for Use of Likeness: I hereby grant Community Lifestyle the irrevocable right and permission to use photographs, video recordings, and/or other media capturing the Participant's likeness (including voice) in any and all forms of media, for any lawful promotional, publicity, or educational purposes. I acknowledge that Community Lifestyle is the sole owner of all rights in and to the media, and its contents, for all purposes. I understand that neither the Participant nor I will receive any compensation for the use of such images, videos, or recordings. Acknowledgment and Understanding: I have read this Registration and Waiver thoroughly and fully understand its contents. I am aware that this is a release of liability and a contract between myself, the Participant, and Community Lifestyle, and I sign it of my own free will.
